Captain?*	* Please enter your team captain name. Please remember to add your team captain as an entrant.
Tel?*	* Please type in the Telephone number of your Team Captain.
Cell?*	* Please type in the Cellphone number of your Team Captain.
Email?*	* Please type in the email address of your Team Captain. This will be used for all correspondence.
Address?*	* Please enter a valid delivery address where your team pack should be delivered to.

Username?*	* Please type in a preffered username for your team. This will allow you to log in and manage your team. E.g. "midrandtoyota"
